Letter Notifying Patients of Physician Leaving Practice

Leaving your practice is a huge decision, and it is important to notify patients of the change. The AMA Code of Ethics Opinion 7.03 recommends sending a letter to patients informing them of the change.



The physician's letter should be professional, include information about the change, and introduce a new physician. It should also include instructions for patients to follow when seeking care. It is also important to provide patients with their medical records.
